Are the forms customizable?
Yes! All forms are fully editable so you can customize them to match your branding, including your logo, colors, and fonts.
What file format are the forms provided in?
Most forms come in Canva Pro and/or Word document format, making them easy to edit. Specific file formats are listed in each product description.
Do I need Canva Pro to edit the forms?
All of the patient guides require Canva Pro, which does require a yearly subscription. Most of our forms are editable via Microsoft Word and/or Microsoft Excel. If you need a free Canva version of a patient guide, please email us at support@theformsvault.com.
Can I use these forms in my Electronic Medical Record (EMR) system?
Yes! Many forms can be uploaded into your EMR. We also offer custom EMR template services for charting and consent forms.
Do you offer refunds if I’m not satisfied with my purchase?
Due to the digital nature of our products, all sales are final. However, if you experience any issues, please contact us at support@theformsvault.com, and we’ll be happy to assist you.
Can I share my purchased forms with others?
No. Each purchase grants one license per practice.

For Canva Pro templates, click the provided link to edit in Canva Pro. For Word documents, simply open and modify the file using Microsoft Word or Google Docs.
Are the forms legally compliant?
All forms are reviewed by our medical and legal team, however it is required that you review them with your own legal and compliance team to ensure they align with your state regulations. We do not guarantee forms are legally compliant for your city, county or state.
